| PROCEDURE AND STEPS
FOR LICENCE :- Licence from Food and Drugs Control
Administration is a must for operating Blood Bank. Under the Statutory provisions
laid down in DCA, 1940, a Blood Bank cannot be run without valid licence u/s.
18(c) of the Act. For the purpose to obtain a fresh licence, necessary steps are
listed below: - Approval of the premises.

Application in Form 27-C
- Requisite Fee of Rs.7,500/- credited under the
correct head of account.
- Documents and other enclosures as per the check
list.
- Joint Inspection from a Team of the West Zone Officers and Local
Officers.
- Adequate machinery's, space, equipment, staff records and reagents.
