a proof of registration of the pharmacist has been renewed

12. True copy of rent receipt for the premises & necessary proof for
the legal possession of the premises like copy of the registered
sales deed etc
13. Rough sketch of the premises
14. Copy of ration card or PAN Card or Identity Card of all partners
or directors & pharmacist
15. Any other documents that Licensing Authority thinks is necessary

of Competent Person - A
person who is a Registered Pharmacist
OR
- A
person who has passed the Matriculation Examination or its equivalent with four
years experience in dealing with drugs
OR
A person who holds a degree of a recognized University with one year’s experience
in dealing with drugs.
