Under the Statutory provisions laid down in DCA, 1940, a Blood Bank cannot be run without valid licence u/s. 18(c) of the Act. For the purpose to obtain a fresh licence, necessary steps are listed below: | | | Approval of the premises. | | Application in Form 27-C | | Requisite Fee of Rs.7,500/- credited under the correct head of account. | | Documents and other enclosures as per the check list. | | Joint Inspection from a Team of the West Zone Officers and Local Officers. | | Adequate machinery's, space, equipment, staff records and reagents. | | NOC from G.S.C.B.T.
